Bandage change: removed both wing and leg bandages, cleaned pin sites with betadine and rewrapped with cast padding and vet wrap. Right wing sites looked clean and free of debris. Left leg IM pin site had some feather debris around pin which was cleared away.

Bandage Change and Fracture Care:
Bandage at L tibiotarsus mostly intact, clean, and dry. Removed the wrap from the bar to facilitate cleaning the area. Fracture alignment appears good, there is no remaining bruising along the limb. The inicision is c/d/i. The skin has separated more medial to the IM pin insertion site, with a scab present. This scab came off easily while cleaning. Cleaned the pin tracts with betadine. Placed two surgical staples to hold the skin in better apposition around the IM pin. Applied SSD to pin tracts, covered with telfa pieces secured with tegaderm. Wrapped bar with 1" cast padding and vetrap. Did not cover incision.
Assessed wing bandage - did not require change today.
Plan to change both bandages in 2 days, +/- laser tx.

Bandage around L tibiotarsal fracture site is intact, did not change.
Removed R wing-to-body wrap and bandage at R humeral fracture site. The incision looks great, c/d/I, no bruising, pin tracts are clean and dry. There is good extension at the carpus, did minimal manipulation at the elbow and shoulder. Cleaned pin tracts with betadine. Applied SSD and telfa around pins. Wrapped bar in 1" cast padding and vetrap. Did not replace body wrap.
Plan for next bandage change on 9/1 or 9/2

Removed the wrap from the bar to facilitate cleaning the area. Fracture alignment appears good, there is very minimal bruising remaining along the limb. The inicision is c/d/i. Cleaned the pin tracts with betadine. Performed laser therapy. Applied SSD to pin tracts, covered with telfa pieces secured with tegaderm. Wrapped bar with cast padding and vetrap. Applied distraction tabs. Did not cover incision. Did not change wing bandage today.
Plan to change wing bandage tomorrow and perform laser tx.

Attempted to flush IO catheter in the L ulna, but the catheter was bent and was not flushing readily. When I applied more pressure to the syringe, p starting vocalizing. Elected to briefly anesthetize to remove the IOC and draw blood.
Anesthetized with Iso/O2 via mask. Monitored HR/RR, no complications, uneventful recovery.
Once asleep attempted to flush IOC again, not successful. Removed catheter and cleaned site with betadine.
Drew 1ml blood from R jugular vein for intake CBC/Chem, no complications.
Stopped isoflurane. Able to change bandages while p was recovering from anesthesia.
L tibiotarsal site looks good, mild bruising normal for this type of procedure. Alignment of limb is good. Inicision is c/d/i. Small amounts of dried blood around pin sites. Cleaned pins with betadine, applied SSD, telfa squares. Covered incision with tegaderm + telfa. Placed tegaderm around IM pin to hold telfa in place and prevent feathers from contacting pin site. Wrapped bar in 1" cast padding and vetrap.
R humeral site also looks good, no bruising, pin tracts clean. Cleaned pins with betadine, applied SSD, telfa squares. Wrapped bar in 1" cast padding and vetrap.
Plan for surgery tomorrow for definitive repair of humeral fracture.

Procedure - Fracture repair - tibiotarsus | Performed by - AMS | Surgery time - 0 minutes | ...
P was placed in dorsal recumbency. The L leg was plucked of feathers
from proximal to the stifle to distal to the intertarsal joint.
Standard sterile prep with leg hung, 4 corner draping, sterile
vetrap over foot and metatarsus. An incision was made on the
medial side of the leg. The gastrocnemius was separated from
the fibularis longus via blunt dissection to expose the fracture
segments. There was no callus present. A 2mm (5/64”) IM pin
was introduced at the tibial plateau and advanced normograde
to the fracture site. The bone fragments were aligned and the
IM pin was advanced into the distal fragment. The muscles were
sutured in apposition with 5-0 PDS SC. The fascia was apposed
with 5-0 PDS SC. The skin was apposed with 5-0 PDS SC. A 1.6mm
(0.062”) miniature interface fixation half-pin was placed in
the proximal tibiotarsus at a craniolateral to caudomedial trajectory
and a 1.1mm (0.045”) pin was placed in the distal tibiotarsus
at a lateral to medial trajectory. Radiographs were taken to
confirm placement. The IM pin was then bent at a 90 degree angle
using a vice grip. The limb was then stabilized in a perching
alignment and an acrylic bar (IMEX Acrylx) was placed connecting
the IM pin and two ESF pins and allowed to cure. TAO and Telfa
pieces were placed at the three pin tracts. The pins were then
cut and a bandage was placed over the pin and bar. P recovered
well from surgery and anesthesia with no complications. The plan
is to rebandage every 24-48hr clean the pin tracts and evaluate
tissues and neurologic function of the limb. Recheck radiographs
in 3-4 weeks. |

Assessment: tibiotarsal fx is fixable, need imaging to better evaluate the shoulder
Plan: -Tx- LRS 25ml SQ Meloxicam 1mg/kg SQ Buprenorphine SR 0.3mg/kg SQ placed R wing-to-body wrap placed SAM splint on L tibiotarsus
-Dx- need to collect AI swab tomorrow morning
Prognosis: extremely guarded due to shoulder injury and change to dull mentation
